The purpose of this form is to provide THON Volunteers with a way to report misconduct that occurs in an official THON setting. Official THON settings include, but are not limited to, committee meetings, THON events, and THON communication channels. Please note that THON is not a social organization; therefore, there are no THON-sanctioned social events.
Misconduct that may be reported through this form includes any inappropriate, discriminatory, harassing, or otherwise concerning interpersonal or organizational behavior that occurs in an official THON setting. Information submitted through this form will be reviewed only by the THON Executive Director, Volunteer Logistics Director, and THON Advisor (university staff member). THON is not a confidential reporting resource. Based on the information reported, THON may be obligated to share this information with appropriate University offices and coordinate with units such as Student Accountability and Conflict Response to ensure the matter is reviewed and addressed in accordance with university policies and procedures.
You may choose to remain anonymous; however, doing so may limit our ability to provide updates regarding the report or contact you for additional information that could assist in the review process. Regardless of whether you submit anonymously, please provide as much detail as possible so the situation can be thoroughly understood and addressed appropriately.
